How do I identify my Moneybox ISA (transfer to Virgin)?

Transfer cash ISA from H.L to Santander.
The Santander transfer form asks for sort code and account number but H.L do not give you one all I have is my H.L client number. My ISA with H.L is invested with Chetwood bank. Not sure how to move forward on this, any advice please.
