I have a copy that I can email you if you let me know your email address.

Thanks for the offer Cralittlewood, I really appreciate that. I won't need to take you up on it as another kind MSE has helped me out already. Does anyone know if I have much chance of getting my cashback using a copy of the form? I notice on it it says they won't, but I'm going to try saying that's what I received?

    Hey Shakka,

I was in a similar situation to you, not having received the cashback form. I did get a pdf copy from another kind MSE and I sent it off with a long cover letter explaining why it would be wrong of them not to accept this copy. On my cashback calculator, it does say that the cheque has been raised, but I am still waiting for it to arrive.

I will email you a copy of my letter once I get a chance to look for it and remove any personal information. Please email me a reminder if you dont hear from me in the next couple of days as I am very busy at the moment.

    Hi Jabi29,

Thanks for your kind offer but I've managed to sort it out already. Another MSE kindly sent me a copy so I think I'm OK. I hope you get your money , it looks like they will be paying you.

I should have learned my lesson from them, I had a different phone with them last year and missed out on one cashback payment of £60 because it arrived 1 day late. I posted it on time(recorded) but it didn't leave the PO until the day after.

    Hi,
I was told by osps there's no need for vouchers anymore, as long as you send the relevant bill & the deadlines have been extended from 30-45 days. I would doublecheck this though!!
No one said it was gonna be easy!

    Hi,

I was also told that the form is no longer required, but when I sent them an email trying to get written confirmation, they sent me a reply with a load of jibberish.
