SAR template needed please

Hi
I am a newbie to this forum and have spent a long time reading the sticky content on the newbie thread to find an example template for a SAR. This is after receiving advice on here on my next steps. Would someone be so kind as to provide a link to a template - I am not sure if this is just example wording or an actual form!

Am I looking for a form template or a template in the way of a few words. Could this be it?
A request for:- ALL photos taken
TIA
- all letters/emails sent and received, including any appeal correspondence earlier
- if the car park was Pay and Display, ALWAYS ask for a PDT machine record from that day, of payments made (VRNs can be partially redacted but insist on getting this; follow it up if they refuse).
- all data held, all evidence they will rely on, and a full copy of the PCN, NTK
- and a list of all PCNs outstanding against you and/or this VRN, and remind them that any claim must be for all PCNs, not several separate claims0 -

Reading a few more paragraphs of that post will give you ideas on what to ask for.
