How do I get another form?

Hi
I have received my form but am unsure which of the two reasons to use as I can't find my original paperwork with the details of the policy. Any clues?0 -

Could you post the address for Canada Square please0 -

Thank you :beer:0 -
I received a reply from Barclays today with regards my old Egg card account for a pre-2005 claim, with an offer of £72. Not as much as I was hoping, but together with the £286 direct from CPP for the post-2005 claim its been a worthwhile endeavour!0

Apologies if anyone has already posted, but has anyone who has CPP via HSBC managed to get a payout from them? I'm about to fill in the form to send to the FSO about it & wondered if anyone else has had any success...0

Yes - it was the same one that someone else posted on this thread. Just wondered if anyone had had any success in making a claim. Mine goes back to 1993!0 -
