I must have spent £30 on various phones in the years I've had the sim, I take it that is enough (as in I don't need to spend it on this phone...?)

As long as you have put £30 of credit on your sim during its active life you can e-mail or phone them and ask if they will unlock your phone. you'll need your IMEI number.

No you don't have to spend it on the phone - but request the unlock with the SIM you have spent the £30 on (can be PAYG or contract)0

I've tried to install this a couple of times but always get a message to the effect that the installation was not completed - something to do with permissions.
Any ideas?

You need to "Application Unlock" your phone first.
Instructions here.
It sounds complicated, but it's not really - just follow the instructions carefully and it takes about 5 minutes.
For step 2 I used the first program on the list (from breaksoft).
