I thought I'd post this in case any London based people using a UK VOIP provider find it helpful:

I use a UK based VOIP provider (Gradwell). Excellent customer service. They also let you port your BT or Virgin phone number to them so you can continue to use that.

If you need to make you own plan, see the post three before this.

Any hints on various proxy settings would be appreciated. I sometimes make calls that connect but are silent - a symptom of proxy problems. This seems to happen when I have not made a call for a while.
