We were talking with OH and we need to get the 'family history' questionnaire i nadvance of our booking in appointment. Our families are in different countries, neither of us are sure of all the heath conditions in our families, and the ones we do know, we don't know in English language. So, we thought that if we got the questions earlier, we could ask our parents, and then have it ready for the midwife and for our future knowledge.
I have tried googling this, but no luck - anyone knows how can I get one without seeing the midwife?...Spring into Spring 2015 - 0.7/12lb0 -
i dont have my notes here at the moment because there on a box somewere, but anyone who's notes are handy will be able to tell you because there is a copy in them, i think its the second page. you have the page for previous pregnancies etc, then one asking about family history.0

Please, anyone who had the booking in appointment - would ou be able to send me a copy of the page?... I don't expect to have the appointment anytime soon yet, as I am still under the care of the IVF clinic, but hopefully, this Friday they will let me book an appointment with the midwife. I'll see if i can find mine later hon. I can tell you the two things they focus on with family histories from other countries, are Sickle Cell, and Thalassemia, which are both blood disorders. Other than that, they want to know about Cardiac (heart) problems, history of Diabetes in you or your family, Anaemia, Epilepsy, Depression, Hyper/hypothyroid problems in the family, Hepatitis A/B/C, if you have had General anaesthetic before, and if so any problems with it, any allergies to medicines, antibiotics etc. If you can find out yours and your OH's blood groups it helps as well.

Here are the family history questions in my notes. There are also questions for you personally but you'll know the answers to those!
You and your partner's family health answer yes or no:
congenital abnormality
inherited diseases
Heart disease/defects
Deafness or hearing impairment since birth
thrombo-embolic conditions - maternal mother or siblings
TB
Diabetes
Maternal family history of perinatal mental illness
Other
Anaesthetic history:
Has anyone in your family had a problem with a general anaesthetic?
Do you know of anyone in the family who has had a problem such as malignant hyperthermia, suxamethonium (scoline) apnoea or severe allergic reactions to anaesthesia?
That's it. There are questions about you, and they go through any previous pregnancies to put in your history, so you need dates of miscarriages, but thats all.

Hi All
I put on last week about expecting. Well a week has gone by and i'm now 6 weeks. I've been really worrying about every little twinge so i am glad to read that (i think) these are pretty normal. Bad period type pains, my stomach feeling as though its being pulled. I'm so anxious at the mo and cant wait to see the midwife, when she finally gets around to getting to my doctors surgery to pick up the new client list!!! I'm finding that my nausea/pain is worse at night and revs up at around midnight and then i keep waking around 4 with really bad period like pain but usually once i've got up and been to the toilet it seems to settle down again.

6 weeks today
Had to go to out of hours doctors last night - GP diagnosed a UTI. Was a bit concerned but my best mate (only person other than DH that knows!) said she had about 7 UTI's during her first pregnancy. Got antibiotics and slowly feeling betterWent to work this morning but had to go home again - hopefully will be back tomorrow or Wednesday.
I do have a question regarding work please ladies - I have my booking in appointment with midwife next Tuesday at 11.30am. I'm not sure what to do about work - I don't really want to book half a day off annual leave but I have recently had a dentist appointment and two other doctors appointments on my calander. I worry what people might think about yet another appointment being on thereAny advice please?

Thanks for reply alizee. When the GP surgery made the appointment I asked if I could have a different time - but Tuesday mornings are the only time the community midwives are in. I'm sure if I told my line manager the truth it would be absolutely fine (and of course, they would be obliged to as you say) but I really don't want to just yet.
