Where can I get the form letter to send to the bank?

Natwest charges £340 (and thats just in one month!)
Offered £266.00
as a goodwill gesture provided I sign a form saying it in 'full and final settlement of all claims I have relating to charges'. They know they have me over a barrel as a disabled single mum on benefits. Like Chrissie there was a problem with the DWP not paying my benefits in. I would like to fight on but not sure I can afford to.
Nationwide charges £380 (again in one month)
Refused point blank to refund or discuss
Debt has spiralled to this amount due to the 'vicious circle' of charges putting me overdrawn in the first place - a D/D for £17.90 was refused which would have taken me less than £1 overdrawn then add on 'unauthorised borrowing fees' plus the fact that as fast as I was able to put cash in it was swallowed up in fees and I'm banging my head against a brick wall. They are now threatening to take me to court as the charges have put me £380 overdrawn and I have no way of paying it.0 -

bagpuss, if you are on disability benefits you may well be exempt from court charges so it may well be worth your while carrying on and rejecting their offer, please read this link http://www.hmcourts-service.gov.uk/infoabout/fees/county.htm
If you decide to carry on and reject their offer, use the following letter
Dear [named sender or Sirs]
Thank you for your letter dated xx/xx/xx
I respectfully decline your offer of settlement and request, once again, that you return to me all charges imposed on this account, totalling £xxxx
I will accept the sum offered only as part settlement and on the clear understanding that I will pursue recovery of the remainder, with a County Court claim if necessary.
My letter before action sent previously indicates that you have until xx/xx/xx to respond before Court action commences. You are reminded that there will be no extension to this timescale.
I trust this clarifies my position.
Yours faithfully0 -
