Guys, did you have to fill and send back some account switching forms to Santander before your switch started?

I received some in the post but didn't fill them in, the switch happened anyway.

It doesn't matter when they are due? So just use the ones you have

I got some paperwork but it expressly said that nothing needed to be done with it.

What does "set up at least 2 active Direct Debits" mean?

Does it count if, on day 60,

    a) there are two active DDs that were already on the Santan account?
    b) two DDs were set up by the CASS?
    c) two new DDs need to be manually set up after the switch completion?

Active direct debits are usually ones setup or taken in the last 12/13 months, after that I guess they become inactive.

I would phone them if you are unsure, then if they give you bad advice then you can complain and get compensation.

That's not what active means. Active just means that they are due to be taken out the following month.
