    I was on Tracker for the whole period you mention and achieved an average of 4.65p/kwh. I live in the East Midlands region which is the cheapest area for Tracker.

From other posts this seems to be normal practice for Octopus i.e. not increasing prices or caps until the fixed term of the tariff has ended and if you're lucky not even then.
    11p pkw/h is the v3 cap
    11p kWh is the v2 and v3 cap
    Difference between v2 and v3 is cheaper standing charge v2

    I signed up to v2 in March and was told that the cap on Gas was 7.5p per kwh but can't find any documentation on this

    The cap on gas for the tracker has never been 7.5p, it was 6p on V1 and then 11p on V2/3.
    That sounds like the low rate electricity price on 'Go', but certainly never been a cap price on Tracker.

    April 9th was the first day I noticed that V2 had appeared, so around then.

    Then if you confirmed you wanted to be moved to Tracker in March (not just asking for more details), you should be on V1 and that is 6p.

    If you have V2 showing in your online account then you may need to follow-up on that with CS.

    Are you sure you are on V2, and not V3?

    No, if you ever want to come off the Octopus Tracker, just email hello@octopus.energy and we will move you to one of our other tariffs right away, alternatively you can switch directly in your online account. Additionally, we have introduced a maximum cap on Tracker prices at 40p / kWh of electricity and 11p / kWh for gas
