Looking at the bumf that they printed for us, it was the "External Transfer In application form (SF215)" and "External Transfer In request form (SF216)" that we needed to complete. Unfortunately I didn't take a copy before completing and returning, otherwise I could have scanned...1

Here are copies of the three pages of the transfer forms sent to me, unfortunately it won't let me upload the pdf file I have so have to upload as jpegs. Maybe check to see if they are the same as ones you can download from NW?

Not sure if this will work... I have attached the above pdf file Nationwide Transfer forms but changed the extension to jpg to enable upload to forum, so download and change extension back to pdf and should see the three forms.This is the form Nationwide sent me to transfer from another bank to them which I filled in all three pages and has been accepted to transfer in June from Charter Savings Bank.I just tried it and it seem to work okay...2

I have requested a transfer pack sent out to me to allow me to request to transfer my KR ISA.
But does this pack also act as a form to open the members ISA?0 -
Try as I may, I cannot fathom whether you can transfer in an existing external ISA from a previous year without transferring ISA money already saved elsewhere for 2021-22.

Yes, you fill the required ISA on page 1 of 2 which shows Fixed Rate ISA, the term, monthly or annual interest and the interest rate.

I've d/l the jpg and changed extension back to pdf - I see 3 forms, but they are 1 of 1, 2 of 2 and a second 1 of 1. I seem to be missing 1 of 2? No you are not missing any, there are only a total of 3 forms, 1 of 2 and 2 of 2 and then another 1 of 1, see the copies I uploaded as jpg..
