    There is a very simple form of wording here on Wikipedia.
    This is the official Deed Poll site and is the site that I'll use when I change my daughters name

    It's not "the official Deed Poll site". There is no such thing. It is the official site of that company, though there is no reason to suspect an unoffical site exists. It is obviously done to deceive people into thinking it carries some legal status. Sharp practice!

    If you look at the guide for enrolling a deed poll at http://www.hmcourts-service.gov.uk/cms/files/deedpolls_notes_0308.doc it gives you a good idea there. It would appear that all that is required is a witnessed declaration.

    I used thelegaldeedservice when i changed mine and my kids names it cost me around £32 for all 3 :)

    Just a quick question, wasnt sure where to ask.

    I just googled Deed Poll and got thousands of services offering to change name by 'official deed poll' for as little as £7.99!

    Are these websites actually official? The web sites all say the right thing (passport approved etc) but it just seems awfully cheap?

    my solicitor charged £35 plus VAT. All it entails is a document with your old name and new name, nothing major.

    i made up my own deed poll...made sure i used the official wording (copied from a website) i found on google. Got 2 witness signatures and way to go!
    Just make sure you get more than 1 copy done..so you can send the banks etc all at the same time

    Hi all

    I am unsure if I need to change my name via deed poll or not.

    I married my husband in 1996 and took his surname, we divorced in 1999 and I reverted back to my maiden name. We got back together and remarried in 2002, but I kept my maiden name, but I want to change it so I have his surname now (pain I know!), but we have kids and I am forever being called Mrs 'married name' and the kids keep telling me I should be 'dads surname' cos I married him.

    To change my name I know I have to go through the whole changing of passport, driving licence, banks etc but can I just start using the married name or would I need to change it via deed poll first?

    Thanks for any help you may have.

    That sounds like a lovely happy ending! No, no need for a formal change of name. Banks etc will change your name with them on sight of the second marriage certificate.
