Speaking of forgetting forms, I've had these questionnaires for 5 months and I keep forgetting to fill them in. The doctor is going to think I'm a really crap parent! (CAST questionnaire for both children, and one about early development for the 6 year old).52% tight0

I have been given 2 forms to take to the hospital twice in the last 6 months - one for blood tests and one for heart tests.
I don't know where any of them are and I STILL haven't had the tests. They're fairly important tests. I KNOW this. I still haven't managed to get it done.
It's impossible to explain to people what the problem is. If I know I should do it, why can't I just - Do It?
