Indeed it was not sent by Lloyds directly.
So far as I can see Halifax/lloyds publically admit to teaming up with "Verve" to enable us the consumer to "Tell us what you think"
....and it appears form a google search that verve have some relationship with Vovici.com.
As others have discovered the email came from Voivci.com servers.
So looks like we are begining to sort this one out.....

I have received one of these yesterday and there is nothing to indicate from the e-mail as to which bank it is related to, if indeed any. The web site lists several banks but again it does not really inspire as being genuine. I gave it a wide berth and forwarded the e-mail to my banks fraud office. If I ever get a reply (hope springs eternal) I'll post back here.~~~~~~~~~~~~~~~~~~~~~~~~~~~~~

This is legit.
I'm a BoS customer and have been using it for a while, also had a telecall about it.
It's a reasonable idea- you sign up (if interested), complete surveys (if interested) and they get feed back!0 -
Notch up another one that's got this email.
It's interesting to note a few things here.- In the email the word "center" is spelt the American way. You'd expect a UK bank to get this right!
- The login page is not protected by SSL (the security padlock). That means your login username and password can be read as it passes through the net or by a trojan/malware on your PC. This is quite a common error.
- On the Halifax sign in page, if you hover over the "Login" link in the top left of the screen it says "Logi-in". So much for attention to detail.
- On the Support Page, under "Other technical error", it offers a link to bankofscotlandhalifax.co.uk for Lloyds. And for Halifax it says "Please direct technical issues with the Bank of Scotland website". You gotta be kidding me!
Just taken another look and the site seems to be down. Hmmm.0 -
Since my post I have been doing some digging and came up with this:

The company lists Lloyds TSB, BoS and Halifax as clients. It's a strange way to go about getting feedback when all my cynical instincts say it's a phishing scam. Banks bang on about giving out your personal details and then foist this on customers without warning. My verdict - inept.~~~~~~~~~~~~~~~~~~~~~~~~~~~~~

I don't even bank with Lloyds... I'm going to assume its a scam and wont be replying.

one from a e-mail address for halifax and one for an address for lloyds

Yes, I got the mail from Charlotte to Dear : . Getting a mail to that address from an unknown sender rang virtual alarm bells. However Lloyds was mentioned in the return address. Quite apart from looking like spam or phish, the url given did absolutely nothing if javascript was not enabled in the browser, not even show a message saying 'enable javascript' (I looked with a browser that deliberately ignores javascript). So the mail has been deleted. Terrible PR by Lloyds group.Eco Miser

Same e-mail from Charlotte received here. BT decided it was spam, which it seems it isn't.
This type of activity is why I shut my Lloyds account. I've since been with Halifax for several years who I have been happy with, but seems that I'm now back with the same bunch of Lloyds fools.0 -
Looks like where in this together. I got one as well but with my full name including my middle name (all correct) which was a concern and got me googling ending up here.
It looks like it does have something to do with Lloyds and their paternership with verve (I am assuming not the champagne mob otherwise I might have been tempted to sign up:beer:). This was part of the email header: "verve_lloyds-eav0tw-0-ru@smtp4" when you go to view full header if you have the option to do so.
