The second one is about putting your affairs in order (so that they're not in a dreadful mess when you die). The first one is about Power of Attorney, which is only relevant before you die.

But Quiet Life's suggestion may get you there. Otherwise it's a case of contacting Age Concern and telling them there's a broken link.
