I've asked for the form three times, to be told they don't have any yet. I feel bad for keeping on asking, makes me feel really money grabbing. Why can't I just print the form out and take it with me to the appointment?

Just keep asking FG... It's your money and you're entitled to it! Also, a friend of the girl I saw today was told by her midwife that she couldn't claim until after she'd had the baby... Her midwife knows even less than the ones with no forms!
A very proud Mummy to 3 beautiful girls... I do pity my husband though, he's the one to suffer the hormones...
Krystal is so smart and funny and wonderful I am struck dumb in awe in her presence.

  • Krystalips - I make notes of things to ask and take along to my midwife appointments - they do laugh at me but at least I dont forget anything.
  • Hmm i can get it. thanks.

We applied for Health in Pregnancy Grant a few weeks back. At my girlfriend's 25 week Midwife appointment the Midwife signed the form and gave it to us. We posted it about two weeks later (after start of April). We received a letter yesterday saying that it could not be paid because it was signed by the Midwife before gf was 25 weeks pregnant. This confused us because the Midwife signed it on the day that my gf was 25 weeks pregnant! GF rang HMRC and it turns out that the Midwife has our due date as 16th July (whereas the hospital consultant has it as 15th July). Basically they said no because it was signed ONE DAY early.

We don't want to ring the community Midwife just to get another form, and won't be seeing her for over a month. Neither will we be seeing the Doctor. We are however going for a 32 week scan on Thursday and afterwards will have an appointment with the hospital Midwives. Would they be able to provide us with another claim form?

somnerd13 - maybe maybe not you won't know until you get there  Your GF could just ring the GP surgery if that is where she usually sees the comm MWs and ask them to ask the MWs to fill out a form and leave it on reception for her to pick up - No guarantees that they will but I know my surgery will do this for MATB1 forms

May do this. She actually sees the community Midwife at the local Children's Centre, which is we she happens to work herself anyway.
  • How long does it usually take for the money to arrive after claiming? I posted my form on the 29th April and haven't heard anything yet? I'm 31 weeks now. Thanks!
If google doesn't know it, how should i?
