  • I have already tried the hospital where my midwife is based and they do not have any yet. Any info would be appreciated.

    Your midwife or doctor will give you a Health in Pregnancy Grant claim form. This will usually be at your next ante-natal appointment after your 25th week of pregnancy.   • Hi Sammi Bee - I have already telephoned the helpline and they have said they cant send me a form but ask that I pass there number into my midwife. Problem is I see a different midwife everytime.

    As it says GP or MW I'd ask the receptionist at the surgery to call for some forms
    worth a try then hopefully you could get the doctor to do it :confused:


    I do love how they do these things!:rolleyes: seeing as we've known about it so long why haven't they been sending info and forms to all the PCTs instead of making us ladies chase it:mad:

    They are on order but they don't seem to have got out to a lot of midwives yet (our manager keeps promising that she has been chasing it up but they say they have a backlog)

    When you do get it filled it by the midwife you have 31 DAYS to claim it in (from the date it is signed)

    Seeing as I have had neither MatB1 or NHS prescription exemption certificate from the number of different midwives I've seen... I won't hold my breath for this form at the end of the month when I next go....
    A very proud Mummy to 3 beautiful girls... I do pity my husband though, he's the one to suffer the hormones...
    Krystal is so smart and funny and wonderful I am struck dumb in awe in her presence.

    Krystal, ask for all the forms. I had to ask for my MATB1 and the prescription form. When I asked for the prescriptions form, the midwife said 'oh you're late getting this aren't you?!' Errrrr, well nobodies bothered giving or telling me about it!

    Don't count on them telling you or giving them to you, we need to do our own homework, cos 9 times out of 10, the midwives will either forget, or not bother.

    I planned to shellsuit but had complete pregnancy brain last time I went and the midwife couldn't get me out of the door quick enough... She didn't even ask if everything was ok or if I had any questions which meant I walked out of the room and 10 mins later remembered everything I meant to ask... And that was the first time I'd seen anyone in over 10weeks too... To cap it all, I saw a friend who is 2 weeks behind me and sees a different Dr in the same building as me and also different midwives... She got her forms last week and has already posted them off!
    A very proud Mummy to 3 beautiful girls... I do pity my husband though, he's the one to suffer the hormones...

    Hey it's not your fault, the midwives should give them out without having to be asked for them - it's part of their job.

    I'm 27 weeks and I've only had one midwife appointment and that's only because I made it (specifically to ask for the prescription form and my MATB1 as I needed that for work/mat leave!)

    I've got a glucose test next week and another scan, so it should be fun when I have to ask for the HIPG form :D

    I have the glucose test in a fortnight... Its ridiculous... I'm at the hospital Monday for the GTT, midwives Tuesday for 28wk check and bloods, hospital again Wed for a growth scan... I'll be hanging around the hospital on the monday for 3 hours... Why can't they just do all 3 things then?!

    Bah anyway... Back on subject... I'm going to write myself a note up my arm so when she goes to take my blood pressure I remember to ask for the forms!
