Hi CC86 - I also joined via Egg and can't believe they are still recommending this company.

Unfortunately they do delay sending out the form, they told me it would be 'quite a while' before the form was issued and when I asked why (ie: it's one sheet of paper) she then backtracked a bit and I got the form within a few days.

I am sure they should show some leniency over the Christmas period and to be fair to them, the post is likely to be bottlenecked at the mo. See if it turns up and if not call them ans ask for another copy? One of them is bound to turn up... good luck!
