I regularly cannot remember things like a name, a surname.

I knew at 10 am I had an appointment 12 noon. I forget about the appointment and missed it.

I go upstairs for something and come back down without it.

I go shopping for an item and come home with other things but not what I went for.

I never used to use a diary. I just remembered everything. Unfortunately, no longer,  but I do not consider myself disabled  by it.
I also have these problems sometimes and like you i also didn't say it makes me disabled. Where did you get that idea from?

There is no rush to complete the PIP form, you can ask for more time. With the Mrs reconsideration I took 2 weeks to answer all the question. I answered them, left them & kept going back to refine the answers (which is why using a computer is a good idea, as it allows for corrections) This allows for better wording of your answers. Rather than the 1st draft.
I know there's no rush as such and they will allow more time but i have a (bad) habit of leaving things to the last moment and rushing them. I know this is bad but it's how i am. I always say "not now, i'll do it later".

Even this PIP claim was due by 3 January and the DWP extended it by their own choice to 17 January. I completed and posted the form on 15 January to get there by 17.

I lose out on a lot of things because of doing them late or not calling a company for an issue i have. Like a bill which may be incorrectly charged and i mean to call them but don't so end up paying it. In my head i say "i'll call them today" but i don't. Sometimes it's small amounts like £5 - £10 and i can't be bothered and sometimes it's larger amounts.

This got me badly in April 2019 with my WCA form which i returned on the date they stated (the letter said 'return by') and they closed the claim the next day on the same day they received the form by special delivery. I argued the point about 'return by' not saying 'should be received by' and they agreed and reopened the claim in July 2021 and should have referred it for assessment but didn't. I then had to complete another form in April 2020 and due to covid i didn't get a telephone assessment until May 2021 which said they need to see me face to face so can't make a decision, then in September 2021 it was another telephone assessment. The DWP later apologised for their error in July 2019 and offered £75 compensation but the matter is ongoing as my claim is that as a result of their error i lost out on LCWRA in 2019 when my eye conditions were severe (I got LCW at the tribunal). The ICE said the DWP should pay another £75. Anway, this is all in my thread about my WCA claim but i say it here as an example of doing things last minute.

Activity 4 - Washing and bathing

    a. Can wash and bathe unaided. - Score 0

    b. Needs to use an aid or appliance to be able to wash or bathe. - Score 2

    c. Needs supervision, reminding, encouraging, or reassuring to be able to wash or bathe. - Score 2

    d. Needs physical help to be able to wash either their hair or body below the waist. - Score 2

    e. Needs physical help to be able to get in or out of a bath or shower. - Score 3

    f. Needs physical help to be able to wash their body between the shoulders and waist. - Score 4

    g. Cannot wash and bathe at all and needs another person to wash their entire body. - Score 8

So how would you word your reply to this section?

From the form the Mrs gets, there is no Yes/No answer?

I said that when my OA is painful i cannot bend the knee to wash lower legs and when i have gout or OA i cannot lift my feet to wash them. She asked if i can bend down or lift them up to wash my feet.

Looking at the descriptors i'd score (a) 0 points because i can wash and bathe unaided but it can take me double the time of someone without my issues and it at times it will not be complete wash.
So do you not think that getting a aid (stool) would be a help?

What would you like advice on at this stage?
You are still talking about your different diagnosis and finding new ones to add.
i think you need to concentrate on each question. How many points you feel you should have got and why.
You are a strong person, your other threads show that you will fight to the end. I think this application has failed, you know it has as well but you can’t let yourself give up.

You're right, i don't like giving up until i can't go any further. Sometimes i win, mostly i lose.

"You are still talking about your different diagnosis and finding new ones to add".

I'm not finding new ones i think, the only new one i've mentioned from the assessment is how much OCD affects me, which i've been honest about and said i didn't realise. The other issues i've mentioned are all i've stated before, as can be seen from my GP record.

Yes, you're right, a stool would help and at times i do sit down (not on a stool). In all honesty i didn't see a stool as an 'aid' and, to me, as aid is a walking stick or handrails etc.

This thread might go on forever. I'm not sure this is helpful for you with all this back and forth. I feel like it's giving you false hope. You seem to be clutching at straws. Forget about the past issues on your NHS record. Just focus on what will get you PI

"This thread might go on forever".

Like my previous thread on my first PIP claim, it will stop sometime.

"Forget about the past issues on your NHS record".

I genuinely don't know what you mean, i posted my GP record extract to show my conditions and that i'm not making up the things i say about them.

I'm going to direct you to the assessment guidance again, because this sort of thing is all in there.  ('Perching stool' and 'shower seat' are mentioned as aids.)

    https://www.gov.uk/government/publications/personal-independence-payment-assessment-guide-for-assessment-providers/pip-assessment-guide-part-2-the-assessment-criteria

    Specifically "
2.1.21 … aids and appliances may be everyday objects, but whether they’re considered as aids in any particular case depends on how the claimant uses the object compared to how (if at all) it might typically be used by someone with no relevant impairment. "  - most people would typically stand to wash or shower, wouldn't even think of using a stool or seat of any kind because they don't need to.

    Also
    https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/pip/help-with-your-claim/fill-in-form-pip/

On the subject of rushing a PIP form, you don't have to start a new claim to be able to preemptively write your answers for a new form.  You could in theory go through each activity with fresh eyes, at your own pace, write up your difficulties with each, and then start a claim (if that's the direction in which you choose to proceed) once you're satisfied with what you've written.

Yet if you read the activity in question. Then you will see the point scoring.

A aid is anything that helps you in your daily life with pip.

If you are going to start a new claim, @Spoonie_Turtle advice is great, do it on a computer & keep rereading it. Also maybe get someone known to you family etc to sit & read through it.
