I haven’t filed or sent anything at all and had no contact whatsoever written or verbal 
I was sent a claim form Friday and now I don’t know what to do

Here's a suggestion...
Follow the guidance you have already been given.

Perhaps start by filing an Acknowledgment of Service following the information in my earlier posts.

You don't need to 'find the second post'.
I gave you a link directly to the second post.
Now read the second post of the NEWBIES thread and click on the only Dropbox link you'll find in that post.

I don't know how you got to that eight year old post.

What is wrong with just clicking on the link I gave you and then looking for the word Dropbox in the post returned to you?

If that doesn't work for you I'll have to leave it for someone else to help you.
