    Who did you phone? The DWP or the health assessment provider?

    The forms are received at a mail sort centre and are then scanned onto the system. It can take a week (or possibly more) to show as received.

    It's returned to the health assessment providers and not to DWP. I don't think they have mail sorting centres for this, at least every time I've returned one it's sent straight to my local assessment centre, I know that by the address on the envelope.

    It's returned to the health assessment providers and not to DWP. I don't think they have mail sorting centres for this, at least every time I've returned one it's sent straight to my local assessment centre, I know that by the address on the envelope. 
    They've now got those Wolverhampton addresses for returning the ESA50 / UC50 too. Or at least the envelope I was given last December had a Wolverhampton address, though the recipient shows as the Centre for Health and Disability Assessments (also known as the Health Advisory Assessment Sevice). I assume it's intended to speed up the process of getting things onto the system rather than sending the form to the local assessment centre.
    The DWP make the mail sort centre addresses available to organisations who provide services for the DWP. The documents are still intended for CHDA and not the DWP. I'm guessing CHDA started using the service in the last year or two. I don't know if the documents are scanned at this point or whether they are sent to the local assessment centre for scanning (which would seem an extra layer of administration). My ESA50 was posted off on 21 December and was shown as received (by CHDA) on 28 December and was sent to CHDA at a Wolverhampton address. The whole of the ESA50 was on the system.

    Hopefully it's just delayed. In case of any problems though, did you photocopy or take a picture of the form?

    Update: I received my UC50 form yesterday, and I got my neighbour above me to post it on the end of our Grove (even though it was hard as I suffer from social anxiety, I had no choice, as I have Agoraphobia)

    But I forgot to add my medical note, what my GP sent me, so hopefully, they will gather evidence by contacting my GP. But all of my current conditions state this and I don't know what it means?

    Does anyone know what that means (Significant, means?) is that mean it's not serious? and will that reduce of me getting LCWRA? I am getting worried now. 

    Did you complete and send it back in the same day?  Did you have any assistance with it?

    I have just phoned again this afternoon and still no sign, so they advised me to call back at some point next week to see if they received it. Also, they said that if it gets delayed, it won't go against me as I am exempt and my benefits won't be affected if it gets delayed. 

    Yes, I took photocopies from my mobile.

    Hopefully next week they'll confirm it is on their system i.e. received/scanned.

    I also hope you made a strong case for a LCWRA descriptor (or more than one) in your returned form. I do have some concern perhaps like others.... but also make sure any supporting evidence gets into play too as they will not ask for it.

    Thanks, yes, I sent all my medical evidence like screenshots of my medical conditions etc on my NHS GP app to this email address (the first email at the top) on this website: Contact Us - Health Assessment Advisory Service (chdauk.co.uk) 

    When I called today, they have confirmed that they have received my medical evidence and they have accepted them which I sent to them in my email address, so, it's just waiting on that UC50 form now :)
