     As the standard DWP letter you have posted contains identifying personal details, you may want to delete it.
    Alice Holt Forest situated some 4 miles south of Farnham forms the most northerly gateway to the South Downs National Park.

    I deleted it, but don't know why it's an issue when it only can contains my name, that's all.

    Unfortunately personal data can get abused and there are almost certainly people who read here who happily would like to target benefit claimants.

    Update: I received my UC50 form yesterday, and I got my neighbour above me to post it on the end of our Grove (even though it was hard as I suffer from social anxiety, I had no choice, as I have Agoraphobia)

    But I forgot to add my medical note, what my GP sent me, so hopefully, they will gather evidence by contacting my GP. But all of my current conditions state this and I don't know what it means?

    Does anyone know what that means (Significant, means?) is that mean it's not serious? and will that reduce of me getting LCWRA? I am getting worried now.

    You have been advised about this previously. Please do not rely on them contacting anyone because it's rare for them to do this.

    Significant.... substantial... very notable... important.... lots of alternative words... but crucially officially recognised and so could be used to help explain/back up difficulties described in relation to some activities in your ESA50 or later assessments etc.

    As advised they will almost certainly not contact your GP for more information.... the emphasis is on you to prove disablements and  entitlement not them... the DWP tend to rely heavily on their contracted assessors of an independent assessment company and they also rarely will seek evidence beyond what is supplied. So if you do get important evidence from someone like GP send it in to the process. Throughout my advising on sickness/disability benefits a theme throughout is to remind it is your claim and to take ownership of that as allowing the DWP or assessors to will often not work in your favour.

    Update: I have just phoned now to chase up my UC50 form to see if they have received it as I posted it last Tuesday (May 7) and they still haven't got it yet. Should it be taking this long? I remember last time I posted it to them and it didn't take this long, I am getting concerned now. 

    Should I be worried at this stage?
