Thank you.  I've had a read through and all looks straight forward on line so I think we'll take this approach (not sure why they said we'd have to make an appointment then  ;) ).

They also said it's just for 'November only' if anyone else is wondering.

We'll apply today, hoping we'll receive card & pin before its pulled!
Maybe they meant if you're coming into branch to do it.

Just out of interest, we've applied now - given a switch date of ...1st December.   Do you think they will honour access to the online Referral Form on/after this date - or is it quite possible they'll pull it off the website given that it's just for November so...tough!!    (not sure if the long lead time is a dirty trick they often use ?).

you can submit the referral form as soon as you get the account number and sort code, there is no requirement that you need to submit this only after your switch gets complete.  My referrals have done this and all have got bonus just after few days of switching date
Did you get your referral bonus too?

yes, just a live example.  Refrred someone, he submitted form on 27th Oct, switch completed on 4th Nov, both of us got £125 today noon.  He submitted the form when it was £50, I think this got changed just last week but still both of us got £125

Any experience with closing/switching out an existing coop acc, and being referred as a new customer for the incentives?

Is there any requirement for direct debits or can I just switch a dormant RBS account I have?

well I dont think anyone has tried this unless they think they wont be getting any referrals.  Personally I am tempted to do this but will be missing out on referring others, losing £125 and earning £125 other way which is not full proof and may not work probably.

To continue, confirm that you: Have read and saved the terms and conditions, opened a Co-operative Bank Current Account or Everyday Extra account and completed a full switch.

I've told my referees to wait until the switch has taken place.

There hasn't been enough time yet. Offer launched around the beginning of this month, 7 working days to switch existing account out, then apply for new account, 3 weeks to complete the switch in (on Coop's online timescales). I guess we'll know in late November / early December if it works... if anyone tries it.
