Leave other dates blank OR put placeholders there as DD/MM/YYYY (so the judge easily knows where to put relevant dates if deciding to use your draft order). What's confusing about that?.2

The point OP is that you cannot put manyt dates in, because you dont know when the order wil be handed down. So you have to leave them "blank"2
