    Leave other dates blank OR put placeholders there as DD/MM/YYYY (so the judge easily knows where to put relevant dates if deciding to use your draft order). What's confusing about that?.

    The point OP is that you cannot put manyt dates in, because you dont know when the order wil be handed down. So you have to leave them "blank"

    I was saying that it looks like a standard six-point draft order and is therefore good to go but you should pay attention to what @Coupon-mad wrote and incorporate that!

    You have THREE claim numbers, of course. 3 CCJ. 
    ONe hour is max length. 
    Judge - DDJ is all thats needed
    Address - read the bit in parens. Youre the D.

    Who should be served with this application? = the Claimant or their solicitors, but not having seen the claim forms the Defendant has no idea if they used different legal firms when they split the claims into three.

    Surely you will be filing it at your local County Court?

    What do the XXXs stand for?

    How many times do you need to be told that?
    Leave it as DD/MM/YYYY for the Judge to decide.

    what do I type next to the annexed witness statement of?
