I've been busy this morning and think I have a solution.
Forget the Post Office - their website is pants - and it looks like they are pulling the plug on those of us with multiple accounts set up principally or solely to satisfy the DD requirements of other banks.
A Santander eSaver, Cash ISA, or a Flexible Saver account allows you to set up as many DD feeds as you require - no limit the cashier at my local branch said.
They printed off a whole load of the Savings Direct Debit forms to do it, and even gave me an envelope to send them to Head Office in Bradford.
It can be downloaded as mentioned in the post by Ed-1
But don't use the one in the link that KTF posted as it has been replaced with a new version with slightly different wording.0 -

You'll need to send paper statements of the current accounts that you are creating a DD for. Despite no mention of this on the form, all of my forms were returned with a note stating this. Same happened with 2 other family members.:grouphug:Official MSE canny forumite and HUKD VIP badge member

Yes you will have no problems.
Just filled out an application, (but not submitted), as on reading Terms & Conditions it says under Section 2 General Conditions:- "12) You may not open more than one Account in your sole name and one Account jointly in your name and that of someone else."

I have just spoken to PO savings and am informed that the only way to open more than one is to open a sole account plus a joint account, so someone is telling porkies.

Last week (or whenever it was), I went here:

Started an application, filled in the details and waited for the confirmation email.
Then I checked the online banking to see if it appeared and did the same again.
As of today it looks like this (accounts renamed):
