Can you get a short letter from the Practice Manager in support of your case at least, saying:
- you had an appointment that day and
- you are dyslexic,
so, the surgery would like the PCN cancelled as a goodwill gesture.
Email that to:

I would do that asap.
It will also help you if PE refuse and proceed with a claim, because you can show the Judge how you tried to get it cancelled and resolve the dispute.
Sorry I only just saw this. When the manager calls me back I will ask her to send me the correspondence between her and PE and will email them. I believe her letter to them did state that I had an appointment and that there was a typo but not sure if she stated I'm dyslexic or not. There is proof on there recorded that I have adhd and dyslexia both of which effect my memory.

I think you ARE being mislead , and should re answer the questions I suggested earlier , as og google streetview may 2016 there are no pay meters on land that clearly state "patient and staff parking , so how do the counsil make money?Save a Rachael

I just spoke to the manager and am waiting for another call back. She said the land and the surgery are owned by the council but leased by them. She was very rude and arguing that it is nothing to do with them as it's the councils. I informed her that I have done research and that the council can't employ parking Eye so who owns the land. She then said it was the surgery manager higher than her who put parking Eye in place, I argued that she could therefor cancel the ticket on my behalf and she got shirty. At this point I am very annoyed and if I have to I will visit the manager in person to get this sorted out..

seeing as the Senior Surgery Manager has employed Parking Eye then the issue with the council was a red herring (Excel run a leased council car park in Chorlton Centre , Manchester)
so if the practice employed these p@rasites , they should have a cancellation clause in their contract as they are jointly and severally liable
the PE invoice should state the creditor (in this case the surgery) , and the surgery should have a contract with the council that allows them to do this , seeing as the council are the landholder
if this person continues to be obstructive , insist you talk to her boss , keep going up the chain and dont let them fob you off , demand a meeting with the senior people and take your evidence with you , stating that the PE system is not up to scratch because it should check the details entered on the machines against the ANPR pictures captured in the car park and not allow an incorrect entry to be made , or find the closest match and offer it to you on screen
point her at the rules issued by the DHS (Jeremy Hunt) on these issues and tell themk it has been this policy for 2.5 years or more

I am hopeful that they will leave me alone now but who knows! I made it clear to her that I will not be paying this fine and like the above comment says it would be a lot easier if you selected your reg from a screen.

So she could email PE, as we knew, the surgery misled you. That is unforgivable, how dare they blame the Council!
PE always supply clients with a 'User Manual' giving a specific 'client contact email & phone number' telling them on what basis/in what circs these fake PCNs can be cancelled. Having a Practice manager mislead you must surely undermine any confidence you have with the practice.
Once the dust settles on this I'd be making an appointment with one of the senior partner Doctors, tell him/her how your confidence in the practice has been totally shattered by this event and ask what action the practice is going to take not only to repair your confidence but also what they are going to do about PE as there will be so many patients traumatised (as you have been) by any entanglement with PE. How does this square with their role as the health care provider for these very patients?Please note, we are not a legal advice forum. I personally don't get involved in critiquing court case Defences/Witness Statements, so unable to help on that front. Please don't ask. .

Google and download the memo they must have missed, which means they are letting their patients down and exposing them AND STAFF to a notoriously greedy and litigious company:

Ask, why did they breach that clear will of Government and let PE in and when does the darned contract expire? Tell them if they renew you will report them to the DCLG.
