I would also note that the Summary bit that ends withDetails of the refund request are at appendix 1.1

Is the bit that goes in your CISAS claim form. So if you were to send the Word document, you would need to remove that section from it (as you've already sent it in the CISAS claim form)
Also, you will need to adjust the document to fit your own case, ie T-Mobile, EE or Orange. The text is highlighted in different colours to try and guide you as to what information you need to keep or get rid of.

Right, I've now found the 24 page word document on RandomCurve's site, so my understanding is a little clearer now.
So once I press submit on the CISAS website, do I then send them that 24-page word document as appendix 1.0 via email, and send my correspondence with EE as appendix 2.0?
As ulaggy said, break it all down,
Appendix 1.1 is claim 1 -1.2 a letter if you have it 1.3 any emails you've sent
Appendix 2.1 is claim 2 apx 2.2 email correspondence etc.
Once you've submitted your CISAS claim you'll get an email confirming receipt. I replied to that email with my attachments which I had saved as PDFs.
I actually sent a follow up email as EE suspended my account that I just prefaced with please see attached further evidence of EEs poor customer practice.
So don't worry too much if the appendices don't flow very well.0 -

I merged all my email trail into one PDF and one appendix ( I had about 15 emails between me and EE)0 -

£50 compensation is paid for EEs lack of duty of care (professional Diligence)/misleading notification/not acting in good faith due to the wording of the text notification and subsequent internet explanation of the effect of the change in the T&Cs.
£50 compensation is awarded to reflect the anxiety caused when EE first used the hidden price clause and the lack of duty of care EE has displayed in both the drafting of the contract and then relying on a term that its legal staff should have known (as per the CPRs) was unfair under the UTCCRs.0 -
Right, I've gone down the route you did of putting them all in a PDF.
When I respond to the email of receipt, should I include anything in that email (aside from attaching the appendices)?

No deadlock reference is needed one of your emails from EE will have referred you to CISAS.
My email was along the lines of please find evidence attached in relation to case xxxxx0 -

Just a quick question for a friend, he sent the email template to EE but they just replied and said I'm sorry you're unhappy but nothing will change, it'll cost you x to cancel, no referral to CICAS.
Where does he go now?

You just need to chase saying your response doesnt address my request.
Page 5 and 6 have some template emails ive drafted which you can rehash to fit your needs i.e.
Sorry can't be of much more help at the moment. But it really is just keep sending emails saying you've not addressed points raised in email of xxx until you get a referral to CISAS.
Whilst it takes a bit longer it does emphasise their poor customer service0 -
I did say at Post #1 that as a "side issue" I am trying to prod Ofcom into taking the action it should be taking to protect US the consumer from the EE change in T&Cs and price rises during the fixed period of the contract in general.
I have now prepared an "official Complaint" to Ofcom which I will copy here, but which can be found in a downloadable format (word document) from the Fight Mobile Increases website.

TEH email to send is FMI-6 and you will also need to attach FMI-4 and FMI-5 (both of which are on the site).
To do the above there is no need for you to have ever contacted Ofcom before and the more people who send the email the harder it will be for Ofcom to walk away from this.
