If you can't get to the bottom of it tomorrow, just download a POPLA form from the BPA website and send it with your printed appeal stapled firmly to it (printed out with a header of the verification code and your name, so that it is repeated on every page). Also with the 'number of pages attached' written on the POPLA form as a cover sheet.PRIVATE 'PCN'? DON'T PAY BUT DON'T IGNORE IT (except N.Ireland).

I phoned POPLA but they didn't know what was causing the problem so they told me to phone PE and gave me their phone number, which isn't on their website. PE confirmed that what I was seeing was just example data (name, email address and phone number) and just to overtype it when submitting my appeal.
So I've submitted it online now but could add additional evidence later if necessary. Fingers crossed . . .0 -

I've had e-mail confirmation from POPLA that my appeal has been successfully submitted and summarising the details I'd uploaded. It's all there but the main body of the appeal is just in one unformatted paragraph. I presume it doesn't get presented to the assessor like that?0

this appears to happen if you copy and paste from word
you can try attaching a word doc, or email them a word doc with your reference and ask for it to be attached to your online submission , just in case they have formatting issues and want a clearer copy0 -
Yes, that's a good idea, I'll do that. I'll probably make a pdf file with the POPLA code and other relevant details in the header and with the pages numbered and just upload that as extra evidence. I might even post them a copy too just to make sure.0

Well I've just uploaded a pdf copy so perhaps I should upload a Word version too? It does say on the POPLA website that pdf files are an acceptable format though.0
