I've read somewhere that some healthcare trust's midwives provide a "tick box" style fprm to use as a birthplan.
Although personally, my MW didn't even bring the subject of a birthplan up with me, never mind give me any help making discisions when it came to writing my own!
Boots have one which is downloadable, which is useful, and to be fair there are a few sites that, if you google "writing a birth plan" will provide a template, which you can adjst according to your wishes.
As for layout, Bulletpoints make reading it clearer, and quicker for those in charge of taking care of you!

I am in the same boat. I have looked on places like babycentre.co.uk and there isn't really anything to print out. I had a look through my green maternity notes and there is a page in there about it.
Basically you kinda want answers to the questions such as (in no particular order):
Where are you giving birth?
Birth partners to be present
Do you wish to be cared for by women only?
Do you plan to breastfeed?
What pain releief would you like?
Are you happy for students to attend the birth?
If labour doesn't proceed would you like your waters broken?
Do you wish to be induced?
What positions would you like to give birth in?
What birthing aids would you like to use?
Does your paertner want to be present in the event of a caesarian?
Anesthetic or epidural in the event of a caesarian?
Does your partner want to cut the umbilical cord?
Do you want a managed placenta delivery (injection)?
How do you feel about foetal monitoring?
Do you agree to an enema if required?
What are your feelings on assisted delivery?
Would you prefer an episiotomy or tear?
Do you want to see your babys head delivered?
Do you want your baby delivered straight onto your abdomen or cleaned first?
Do you want your baby to have vitamin K injection?
I am sure there are tons more but they are the ones I can remember I have recently been thinking about with a view to writing my plan. Your midwife is supposed to go over all this with you at your 36 week check up anyway but the midwife is usually so busy you only really get 10 mins at a time! My advice is to research all the pros and cons yourself on the internet and make your own choices and if you have any speicif questions ask your midiwfe. She wont have time to go through all the pros and cons of every decision with you.

It really is difficult as i dont really have any strong feelings either towards or against some things.
I have made a start by copying some sample ones into word and just chopping bits out and changing words until it sounds like me. I like the idea of it being very simple with bullet points rather than a ten page essay like some i have read :eek::heart: I love my gorgeous little girl0 -

Then don't decide - just see how it goes. When I had my first I wasn't sure whether I wanted him delivered onto me or cleaned up first. As it happened with both of them they were both taken away for medical intervention anyway so it wasn't up to me in the end. Not been on for a while.
Had an antenatal appointment today and the midwife is sending me for another scan as bun isn't as big as he should be for 34 weeks, so might not be getting my homebirth but i gotta do what's right for bun.
had a feeling all weekend i would be sent for a scan as i was huge with DS, another thing that concerned mw was that DS was 8lb 7oz and bun is really dinky, trying not to get too worried though, bun is really active, especially at 4 in the morningIf everyone cared and nobody cried, if everyone loved and nobody lied, if everyone shared and swallowed their pride then we'd see the day when nobody died.

I have basically written that all i want is the baby delivered safely and healthy, that i trust the people delivering the baby and am flexible about the preferences i have written and thats all they are.
I know lots of people do have strong feelings regarding their birth and i hope i dont cause any offence by this post but i have had so many problems throughout that i just dont care how she gets here as long as she is OK.:heart: I love my gorgeous little girl0 -

Yes my birth plan says 'decide at the time' for a lot of it because god knows how i am going to feel. Who knows what position i will want to be in? Plus I am fully prepared to throw the whole thing in the bin and go with the flow. Dont get too bogged down with what you absolutely want as if things are taken out of your hands you may be stressed about it. Just try and work out which things you really want and concentrate on those and everything else will fall into place behind them. Or not. Either way it doesn't matter. SO long as the babies are happy and healthy :j0 -

How much discharge do you have?? I mean since I got pregnant I have had a fair bit - I wear pantyliners and it seems fine.
BUT over last day or so this has just gone mad. I can feel it leaking out all the time and i am contantly wet with it (sorry!). I even sneezed earlier in the car and a huge blob came out I felt it! It's normal colour and I am not in any pain so I dont think I have an infection or anything but i cant find much about it increasing on the web? I am 33 weeks now.0 -
