Yep, I have just received mine too. :T

It has been 3 weeks since I signed up, and I have had 5 DVDs in the post during that time. I have been pleased with their service in general, although not so impressed with their online player. But all in all, a good free trial with a £20 Amazon bonus!

Your's should say 2012, so you've got a year. At least mine did. Still didn't receive my dvds or the form to report them missing, though. <sigh>

Ahhh year cheers, I just saw the date and assumed lovefilm were trying to screw me over.

If you ring them up (find the number on the site then no to 0870 it) then they should sort it out. I had about 5 blu-rays then cancelled two weeks in to be safe.

Just a point that if you go here: https://www.amazon.co.uk/gp/css/gc/payment/redeem-gc-balance you can add the gift card balance to your account without actually spending it, this way you don't have to try and remember/find the code when you want to buy something off amazon.

I did contact them, they told me it takes 4-8 weeks to get the form from the Post Office (and that's if they ever really requested it, which I'm skeptical about). Hopefully it will come, and hopefully they will let me cancel membership before it comes, otherwise I will have to pay for membership for nothing as I cleared my rental list after this fiasco -- can't risk any more discs not arriving (or not making it back to lovefilm when I try to return them). I called the Post Office, too, and they said they only send out this specific form (P91) if the sender makes a claim -- meaning there is nothing I can do if lovefilm doesn't request the form.
