    1. I can't find the reference I had about this but somebody else may be able to confirm... The form specifically instructs the GP not to contact you before completing and to answer the questions based on observations already made and information already received. If your condition/s have changed/worsened since you last saw your GP then obviously what they write won't confirm what you've said. It's therefore very important that you go to see a GP at the practice you're registered with and discuss this before they get the form!

    2. Yes you can provide more information. You can ring the DLA number and talk to them about it all. I badgered a friend into doing this about a month ago when she suffered a sudden deterioration in her condition only a couple of weeks after sending off her form. If they haven't yet made a decision then they will use all the available information.

I think I will phone them then but they wont get it till tomorrow.

I didnt put my gps name I just put the practise down as I just didnt know who to put because of how rubbish mine ios and when I went to the last one she was the one that told me they hadnt had anything back from tne hospital consultant and they had, I know I will have to change gps soon. Last time I went though I first stumbled on the way in the room and fell over as I got up to leave so she had to help me up and out so she must remember that and she surely would look through all my hospital notes before replying to the dla

That's good - but go along and remind them, then ring the DLA and give them the name of the GP you actually saw...

  • Filling out a DLA form is never easy especially for peope who suffer from mental health problems,

Putting a few peoples minds at rest, I've not seen my GP for over seven years, When filling in my last DLA renew where it asks when did you last see your GP I put better ask him/her that as I've never seen them :D

I suffer from Agoraphobia so I'm unable to visit the GP and they never seem to want to do home visits,

As for DLA if they want you to see a doctor you can request a home visit, I always request a home visit well I would do as i'm unable to get out,

I'm hoping to help out where I can with DLA and people who suffer from mental health problems, So any questions ask away.

After getting the dla form posted I was ok once it was done and thought whatever happens happens and even though the ICB medical form IB50 came in the post the day before my granddaughter posted the dla form I thought if I can fill the dla form in the IB50 one would be easy and I was fairly confident but then I started to read things about dla home medicals and the ICB medicals and now I've got really worried about it all. I'm convinced its all going to go wrong as it seems that some of these doctors dont even put the truth down sometimes. I don't know if I've filled the form in right and keep thinking I've missed important stuff which I'm sure I have. I was going to ring them to add stuff on but I dont know if I should or not.

I'm worried that I'll lose the ICB now and I know I wouldnt even be able to get to the job centre without a car or taxi to the door and it doesnt seem right that I'd need to do that to get there aswell as all the other reasons.

Do you think I'll more than likely be asked to go to a medical for the ICB? as I'm going to put on my form that I want to request a home medical if I do have to have one, is that the right way to go about it?

The IB50 form has to be recieved by them by 8th April do you think I should send it soon or wait for a bit in case the dla get back to me? They should have recieved the dla application last Friday 27th Feb

Is it highly likely that I will have to have a home medical for the dla too seeing as this is a first application. It said on one site that these doctors arent really independant that they are actually really the DWP

Its only been a few days and I bet they havent even looked at my form yet so I'm going to be a total wreck by the time this gets sorted if it ever does at all. I don't think I could deal with a tribuneral or an appeal
    Thanx

You need to go ahead with the ICB forms, the DLA, usually take at least 8 weeks to go through the system, may take longer if its your 1st application, it really depends on what info they need, when I initaiily applied for mine it took 4 months to go through, but I had to have gp report and independant doctor come out to home.
I'm the same with the forms, did I put the right thing, I should / shouldn't of said that, I wouldnt worry, just fill in the form and honestly as you can and see what happens from there.

When I claimed DLA recently , I didn't have to go for a medical. They asked my GP for a letter, and the award was made within 6 weeks of me claiming.
DLA and IB are completely separate. They need to ask for enough information to make a decision. If you're asked for a medical it doesn't mean they don't believe what you've said. They just need further clarification that you do or don't qualify. It's just unfortunate for you that both thing are happening at the same time.

I haven't had a medical for either of my DLA awards. If they have sufficient information and evidence there is no need for a medical. (I have ME, one of the more 'difficult' award categories.)

I got a letter from dla today confirming that they have recieved my applicaton and are looking into it, they said they recieved it on 27th (a week today) is that quite quick? It says if I havent heard within 8 weeks get back to them, whats the average?

Phoned the helpline again about my ib form they said its ok to type out the info on an a4 paper and send it with the form rather than write in the info boxes but I must obviously do the tick boxes and all the rest of it on the form. They said to explain about the need for a home medical visit on the form too so they will get it with my form.

Have to say having rang the helpline a few times for both dla and ib at separate times I've noticed they are much nicer if you are asking about dla rather than ib

the 8 week is normal, as they need to assess the info and incase they may need to contaxt anyone else, and the othe thing to remember is they mayhave a backlog so in theory u could sat in a pile of papers for a week before they get moved.
