Yes its a good idea about just giving them that sheet to write on and not the rest, the back of that page is the bank details so I can still fill that side in and print it all off and just give them that sheet. Trouble is though what she doesnt know is quite important to go on that statement bit, I might ask my sister as she knows but she doesnt live near me but she does take me out from time to time to get me out of the house.
There are things that I would need help with but have to cope with in the only ways I can but it does say struggle with or get help with so I have still put them down and how many times a day will this be ok?Thanx

yes, you put down everything you struggle with regardless of whether you get help or not!
with regard to the supporting statement - you can ask more than one person! and it doesn't have to be on the sheet, it can be a separate letter.
additional information - you can include information on what you can't/don't tell people and why, or anything else which you think may be relevant. if you forget to put it in the form then put it in a separate letter or ring up afterwards!Eat food. Not too much. Mostly plants - Michael Pollan

Does anyone know if you can or have to repeat answers in questions on this dla form? I mean like on question 27 28 29 and 30 there are the same reasons and other reasons and now on question 31 getting around outdoors and having someone with you. the reasons are the same as earlier questions and more on top but should i repeat answers and and add more or not. I just didnt want the person reading it to get bored of reading the same stuff and not bothering to read the restThanx

Yes you can, Lady K. For most of us, numerous things have a similar effect on us - pain, tiredness, loss of balance, etc so if the same or very similar thing happens when you (for instance) get out of bed, try to go to the toilet, try to get washed/showered/dressed then put it down. As many times as necessary. If you put different answers then someone might start wondering if you're making things up. It's reasonable to expect the same or similar responses to many things that make mobility difficult, for example.Does anyone know if you can or have to repeat answers in questions on this dla form? I believe you previously said you'd downloaded the form so the easiest way is do a copy paste exercise & just change or adapt as needed. I'd be worried they might not bother looking back if you write in as "as in number..." This is what the welfare rights worker advised me too. And I got the award we expected so think it worked.0 -
Yes I am copying and pasting which is much better, I really don't know how people do it just on the form its sending me crazy. Only thing is its taking up room as I'm adding more each time so the text might turn out really small when printed even though I've expanded it online. but I agree I had already decided the same that the reader may not be bothered to go back and re-read it all if I just put the numbers etc. I'll be so glad when this is done I just hope it gets acceptedThanx

Is it ok to put more than one amount in how many times a day like 3-4 times a day or or 2-4 times a day rather than 2 times a day or 4 times a day as sometimes it varies?Thanx
