That sounds like either:
a) at the end of each row, you missed out the last stitch
or
b) at the start of the row, you skipped one stitch and inserted your hook into the stitch next to it...
Here is a link to a tutorial about single crochet stitches with slow motion parts. If you want to check out whether you are starting the new row correctly, she covers this at about 2:45

Ellie, if you are having trouble making a really flat circle I would highly recommend following this:

I think I've worked out what the problem was... I was trying to work into the top 2 stitches as the work is facing you, not into the 2 stitches of the side chain! It still looks a mess but at least I know what I'm supposed to be doing now. Also I think that counting is imperative, mine looks such a mess I think I'm trying to work into things I shouldn't be!
