Just received my card - I applied on the day I posted this thread (3 November) and received it on Monday this week (6 December) so it does take a few weeks to arrive.
I too am bemused by the little cut out bit on the side of the card - no idea what that is for!
And Spikey I would say that the form needs to be filled out in the name of the disabled person. Any queries the card people can be e-mailed at info@ceacard.co.uk0 -

Q. Why has this scheme been set up?
A. To introduce a voluntary consistent approach to the provision of concessionary tickets for carers across the cinema industry.
Q. How do I apply?
A. There are two ways to obtain an application form. Either print one from this website or collect one from a participating cinema as listed under national chains, or independently under post codes.
Q. Once I have sent my application form off in the post how long will I have to wait for my card?
A. This will depend on the following three things – the time taken for the application to arrive by post, the volume of applications being processed at the time, and the time taken for the card to arrive back by post. In general, we estimate this should be three weeks. If you do not receive your card after four weeks please contact the Card Network Team who will investigate it for you.
Q. Whose name and photograph appears on the card?
A. The card is issued to the person who requires assistance to enjoy the cinema. In this way, there is no restriction on the accompanying person, whether parent, family member, friend, care worker and so on.
Q. How long is the card valid for?
A. Three years from the date it is issued.
Q. Why is there a £5 fee to pay?
A. This scheme is unfunded. Every effort has been made however, to keep the processing fee as low as possible.
Q. Can a cardholder have more than one card?
A. Yes, but please note the processing fee will apply to each application.
Q. How many photographs should I send?
A. One clear photograph. Please do not send them with the word PROOF stamped across the face.
Q. Why does the application form ask you to write the name of the applicant on the reverse of the photograph and also attach it to the form?
A. If the photograph becomes separated from the application they can be matched back together easily and quickly.
Q. What proof should I send?
A. 1. For Disability Living Allowance – copies of letters, statements, or benefit books (first two pages) confirming receipt of the allowance for the person named on the application.
A. 2. For registered as a blind person - copy of certification.
A. 3. For holder of a disabled persons rail card – copy of the front and back of the card (national scheme only please not a local travel pass)
Q. Can I send original documents and have them returned to me?
A. No, please send copies only.
Q. I am not sure about sending personal details concerning a family member or myself. What will happen to them?
A. Once we have verified the entitlement to the scheme the qualifying documents will be destroyed and securely disposed of. No file copies of documents will be held.
Q. What is meant by “fold to A5 only”?
A. The application form is one sheet of A4 size. One fold across the middle makes it A5. Too many folds could make the form difficult to process or damage the photograph or payment.
Q. Does the card have any particular features to be aware of?

My wife receives DLA at the higher rate. My daughter decided they should visit the cinema, I told them to take my wife's "disabled" parking card and ask if there was a concession. The carer was admitted free. If this is so, why pay £5 for a card ?.
'If the person visiting the cinema requires a carer to facilitate that visit, then the carer can obtain a free ticket - just tell the box office employees before you ask for your tickets. Currently no further ID is required but

My friends son has Autism has they get DLA for him.
Do they apply in his name (son's) or their's (parents)?

Used my card for the first time on Saturday. Decided to book tickets as I had already spoken to Odeon who said this was ok. The girl on the phone didn't know what a CEA card was, so just went on to book normally. Felt a bit miffed, so emailed Odeon about it, and they emailed back straightaway, saying they were going to sort it out. In less than an hour they had cancelled the tickets booked and re-booked with the free ticket. All we needed to do was pick the tickets up at the cinema and show the CEA card. I have to say that Odeon were very helpful and effecient, they just need to educate their staff on what a CEA card is.Me transmitte sursum, caledoni0

I am thinking of applying for the card but have a few concerns hopefully someone will be able to answer for me
1. Do Cinemas let you in without the card for free? (could prove DLA or Carers if asked) the reason is if they do why do we need to purchase the card.
2. If you book online how do you book a free ticket?0
