    Section one: 'Did you receive of a wavier of the ineligibility?' is the question referring to the visa waiver program online (authorised/unauthorized to travel)?

    Section two: Location & type, the guy was just reprimanded, do I put the police station down or the place of the offence?

    For Sentence, Not applicable, right?

    Does the supplemental nonimmigrant visa application, form DS-157 need to be completed? (link http://www.usembassy.org.uk/cons_new/visa/niv/add_males.html)

    The above form the right form for a police certificate? as it states on there immigration visa, when my friend is applying for a non-immigrant visa.

    It relates to being found ineligible for a visa, and then applying for a waiver.

    If he has never applied for a visa, the answer is "no".

    I would just put the town/city down, and the in the sentence section, type "reprimand".

    There aren't any trick questions; as long as he fills it out accurately and to the best of his ability, he'll be fine.

    If he's coming here as a visitor, then yes.

    What if he applied for visa waiver online which was said not authorised to travel and now he is filling the the visa form for the first time, is the answer still no?

    Visa waiver isn't a visa; the clue is in the name. ;)

    No they don't email you. A couple of mins after completing the ESTA you get a message on the screen whether you are approved or not.

    It's a good idea to print it out once you are finished as the reference number is quite long and complicated with a mixture of numbers and letters.

    It is valid for 2 years and you may want to go back and change flight and accommodation details even though these details are not compulsory.

    'A supplemental nonimmigrant visa application form DS-157 (Completed by males aged 16-45 and all applicants aged 16 and over who were born in or hold a passport from North Korea, Cuba, Iran, Libya, Syria & Sudan).' He is between the age range and born in UK, does he have to fill in this form?

    'Evidence of sufficient funds to cover all expenses while in the United States' One Bank statement to show work pay and balance?

    'Evidence to show that the applicant has a residence abroad to which he/she intends to return at the end of the stay in the United States. This is generally established by evidence of family, professional, property, employment or other ties and commitments to some country other than the United States sufficient to cause the applicant to return there at the conclusion of his/her stay. Applicants resident in the United Kingdom will be required to show strong social and economic ties to the U.K.' Return flight ticket, enough?


    I will be SHOCKED if after all this my friend can't go!

    'A stamped, addressed Royal Mail Special Delivery envelope for the return of the passport. No other form of delivery is accepted. You must keep a record of the Special Delivery reference number as this may be used to track the package online at https://www.royalmail.com once the passort has been handed to Royal Mail. One envelope is sufficent for up to 4 passports;' When you go for the interview, don't they stamp the passport there and then?

    No they don't. Assuming they approve it on the spot, still allow up to a week for the visa to be issued and the passport returned.

    It says:

    The visa application form DS-156 which must be completed on line before printing. As well as the printed hard copy of the 3-page DS-156 with barcode, applicants are advised to bring a saved copy of the form on disc.     Males aged 16 to 45 are required to complete a supplemental nonimmigrant visa application, form DS-157. This form must be completed regardless of nationality or place of residence.

    Why not take more to show consistency - that he hasn't just had a friend dump some money into his account temporarily?

    I would not have thought a return ticket would show strong social and economic ties to the UK.

    I appreciate that you are trying to help your friend out here - but as someone who went through the visa process to come to live in the UK (I know it's a different thing, but general principle is the same) you want to be sure you have sufficient documentation to make them happy - bring more, not less. I understand that it may be that your friend doesn't even need a visa - if I remember it was pointed out that if he was under a certain age when he had his run in with the law, it wouldn't prevent him from going through and using ESTA...but it seems to me that you have to ensure you've got enough documentation - so why worry about trying to find out what's 'enough'?
    Does remembering a time that a certain degree of personal responsibility was more or less standard means that I am officially old?
