Download the PDF from MCOL, fill it in and email it ASAP, after checking your MCOL claim history ASAP first

Do you know where I can find info on how to complete the questionnaire? My sense is that I just need to state it's fine for the small claims track and that I agree its suitable for determination without a hearing?
