This is my SAR email according to #2. Is it correct for my case or do I have to add my address to it if I have attached my V5C?

Subject access request ( Data Protection Act 2018 / General Data Protection Regulations (GDPR) ): I have attached my V5 for your attention.Please supply the data about me that I am entitled to under data protection law relating to myself.
- ALL photos you have taken
- all letters/emails sent and received, including any earlier appeal correspondence- all data held, all evidence you are going to rely on, and a full copy of the PCN, NTK to me
- a list of all the PCNs outstanding you have against me, and please remember that any claim against me must be for all the PCNs you have against me and not several separate claims.If you need any more data from me to confirm my identity, please let me know as soon as possible. It may be helpful for you to know that data protection law requires you to respond to a request for data within one calendar month.
If you do not normally deal with these requests, please pass this letter to your Data Protection Officer, or relevant staff member. If you need advice on dealing with this request, the Information Commissioner’s Office can assist you. Its website is ico.org.uk, or it can be contacted on 0303 123 1113.

Don't prioritise the SAR. Do that in April sometime.
Defence is the priority and if this is DCBLegal then you just adapt the defence by Johny86 as I already advised:You will be simply adapting the recent defence by @Johny86 - which will take you half an hour max. But when you admit to driving, the POFA means nothing to you. That POFA NTK point needs removing.
I take it what you've shown us is actually paragraph 2 then paragraph 3, despite your numbering?
Remove Johny's para 4 and re-number the rest.

That letter you mentioned in your first post was the notice to keeper, however what @B789 is pointing out is that you state you were the driver so it is irrelevant about no notice to keeper as that only helps if you are defending as keeper and avoiding having liability transfered to you.5
