    I have found an incredibly useful document - from last year
    It's my medical retirement facts (reasons etc) - from the OH company who agreed it - "HML" Health Management Ltd - a company who sift PIP cases - it gives all of my medical history in one document !
    it ends Conclusion
    "Based on the available evidence from his treating GP and specialists from orthopaedic, urologist and speech & language therapist, ICRT, it has been apparent that he had struggled in his role for a while. He had difficulties in all aspects of maintaining his activities of daily living including mobility, balance, dexterity, speech, and communication."

    Suggest add in as evidence. If it, or any other evidence you'll supply, has any specifically relevant detail regarding any activity perhaps refer to it in the appropriate activity section when describing problems.

    Would the followng be a good thing to put in 13a (Does your condition affect you planning and following journeys?)

    I would be unable to use public transport - due to being stuck in wheelchair 

    I can (and do) drive – but I need help during a journey when I’m not driving -
    for example, when I try to get from my car to where I’m going

    Also – I am unable to refuel the car myself – because I would need to lean
    against the pump for support to physically fill the car with fuel and I would need
    to walk to the cashier to pay – which I would be unable to do


    I also plan to include photos of all of my aids and equipment in use

    • ability to leave the home and embark on a journey and
    • ability to follow the intended route once they leave the home

    This activity is designed for limitations on mobility deriving from mental health, cognitive and sensory impairments, whereas activity 12 is generally designed for limitations from physical problems. Cognitive impairment includes orientation (understanding of where, when and who the person is), attention, concentration and memory. Any issues with the ability to stand and then move are not applicable under activity 11, but under activity 12.

    Your numbering is askew - 14 is "moving around" and 13 is "planning and following a journey"
    Doesn't really matter - as I earn 12 points through MOVING AROUND (14)
    But according to CAB - This is a relevant actvity

    If you can drive  

    If you can drive you need to make it clear if: 

    • someone else plans your route
    • you can go out alone in your car
    • you can only drive to places you know
    • your doctor has advised against you driving 
    • your medication affects your ability to drive
    • you need help during a journey when you’re not driving - for example, when you walk from your car to where you’re goin

    Yet again you are misunderstanding the whole activity and we are again going round in circles. What you've quoted there doesn't relate to a person that has physical conditions! I give up!

    I appreciate that you are trying to help but you obviously haven't even seen the latest PIP2 form which is worded more in favour of a non mobile individual
    Ie) tick here if you CANNOT walk 20 metres WITHOUT aids!!!

    Agree with Poppy... to me (and I caveat that I do not know your full circumstances), the problems you describe and the cause of them do not fit into the context of activity 11 because they appear unrelated to mental health, cognitive or sensory impairments but they do fit in to the context of activity 12 where hopefully you do indeed get awarded the appropriate points by the DWP.

    The CAB advise detailing problems associated with a journey involving driving because mental health, cognitive or sensory impairments may make the planning or following a route difficult. 

    Of course there is nothing to stop you detailing your problems in activity 11 but I suspect if you honestly do they will consider them represented in activity 12.

    On the numbering and the questions on the form. Once again I remind you these questions do not necessarily translate to the descriptors... the form is to gather information. You have been given extensive information.

    Yup i have seen the new PIP form. You are twisting things and trying to make those descriptors fit you and that's not going to work.

    And where did you get your activity numbering from? 

    The questions on the form are worded differently to those in the link i posted above which was the PIP assessment guide for assessment providers, which again proves you don't read the links that are posted. If you did then you would have seen where the link was from.
    Yes, i read the link and as advised, it doesn't apply to you. It applies to someone with mental health, cognitive and sensory problems.
