Please can I ask..  do you have to call up Halifax to choose the £5 a month reward option?  or can we choose on our online account?     Thanks for helping
If an online option was available, few would have chosen to call.

    Hi alph,
Provided your account was opened after 13 March then:
    call 0345 129 9711, or 0113 369 7711 from abroad (lines open 24 hours a day, 7 days a week), on or before 12th June 2020.
If your account is older than 13/3 then unfortunately you already missed the deadline.
BUT as many commented here you may be able to upgrade the standard account to reward account ONLINE from tomorrow. So, not a biggie the miss if this is correct.
    Good luck!

Since an individual can have two accounts that pay the new reward (according to T&Cs one sole and one joint), the phone robot would offer to let callers change both of them on the one call. Therefore nothing can be deduced from this about what will happen for those people with multiple sole accounts.
At least one person on this thread said they registered more than two accounts on one phone call. I can't remember if, like me, they had 4 or just the 3 that were allowed way back when.
I did my 4 on separate phone calls, hoping to beat the system...if indeed there is one!

I was one of those, as I registered 3 accounts.
However the silly robotie accepts anything you tell it, even non Halifax accounts and presumably non existing accounts too!
Given the little trust I had on it, I called a few days later and informed all 3 accounts again.

In the Halifax app, if you press on the 3 dots next to your account, their two of the options are "Add your Reward Extras" and "Track your Reward Extras"

AFAIK, this only works for the Ultimate Reward. Certainly doesn't work for my Reward

I don't want to speak too soon but, for all 3 of my Reward accounts, when I click on the "Track Your Rewards Extras", I now get a message saying that, as I've just added rewards to the account today, it will update soon, so hopefully the registrations have gone through successfully

Worth noting this from the FAQs for anyone expecting a Reward to come through at the start of July:
"If you’ve done all the things we asked of you at the end of the month, you’ll get your reward around the 12th of the following month."

I am seeing the same but only in the Halifax app. Online banking doesn't show "Track Your Rewards Extras", it just lets me "View account benefits" and shows the old £2 information. If I access my Reward account via the BoS app, I can't see any reward information at all.
