As you haven't yet successfully claimed ESA then this is the wrong form. To claim ESA you need the ESA1 form. The health assessment providers send you the ESA50 form after you've successfully claimed ESA, as stated in my previous comment.

We have had this ‘I can’t type on the form’ PDF argument before on THIS THREAD.
The OP is either a WUP or isn’t reading anything that’s shown to him. No point helping. Let the CAB do it as they clearly have it all covered.0 -

My first post on this thread was this morning, I hadn't even read the thread as I could not give any information on the OPs query.
I was awake during the night and spent a bit of time? reading all the posts, all I see is someone who does not want to be helped, despite very many helpful replies, actually the poster reminds me very much of a previous troll, if not him then maybe a relative.0 -

It seems to me that the OP should make a claim - if he's turned down then he can appeal - if the appeal is turned down then he pays up if he wants the higher SP.

That is not the correct form for "Legacy / old" ESA - Which I am trying to claim
This form is the "New" ESA - which involves my wife - "Legacy / old" ESA is a standalone claim
Legacy / old ESA is what CAB advised me to claim !!0
