I can't be bothered digging out the forms to provide the specific examples that you crave. I've completed both forms in the last eighteen months and the likelihood of a catastrophic mistake on the EHIC application is virtually nil. The passport one is more complex.
You cheerfully carry on talking up the joys of paying £35 for something you can get for free if you like. Not very MSE but perhaps provides a niche solution to the current situation with the official web site. I can't see it myself.0 -

A very costly mistake would be to travel without a card and require medical treatment. That's the point of this thread, and my posts are consistent with money saving.
They are free and the application - as mentioned by more than one poster - is very simple and requires no checking.

When you think that paying £35 for something that is free is OK.....I don't think so.
I do think that even if MSE told you that the site you've mentioned is a shyster site you'd still try to defend it.0
