    Sorry for the double post but I just wanted to say good luck at the dr's, I know your appointment has already passed but I really hope she's finally sorted your medical form out for you.

    And she will need to have all the right bits of paper to back up her decision, if required or in case that one is the one selected for their procedures to be checked by their internal audit department.

    Oh no!!!! that's outrageous!! I hope they told the receptionists off!
    Don't know if this is too late to catch you but make sure you keep the original or at least take a copy of this for your own records

    That was your post on Monday - so the secretary at your Dr's was unaware of what had happened to the form?? I would consider having a few well chosen words with the Practice Manager about this matter - it was vitally important, and the HLO could have made her decision last Friday, and you could have been allocated a new home by now, if the practice staff had realised what was going on!!

    Jeez MOT the way you have been treated defies belief, yes I know things move slowly but your journey to getting declared homeless has been full of incompetent idiots and stupid time wasters all passing the buck from one to the other:mad: :mad: :mad: :mad: is nothing ever done simply these days, I have a friend who will be in your situation soon, and Im getting worried now that she will be treated the same.
    My local council is swapping over to this "Choices" form of letting empty properties, and it looks like the more persistant your are the better your chances.
    I so hope this comes to a satisfactory conclusion for you...will keep watching your posts.
    I wish I could wave a magic wand and make your probs and others like your situation be sorted out, unfortunately lifes no that kind.

    Hi all,

    Well the deed is done and I am now in the hands of the HLO:confused: . She was taking it straight to the Council Medical Advisor for their comments on what my dr had written.

    Gforce I did scan it first before I took it in!

    The Dr reported that I am suffering from depression and anxiety due to work stress and marital breakdown and that I am on Fluoextine 20mg and beta blockers.

    In answer to the council's question 'In what way does your patients condition affect daily functioning/activities?' she replied 'Recurrent panic attacks, tearful, poor sleep, unable to take on any new situations, lack of confidence - leads to confusion in dealing with finances'

    Council also asked 'Details of any relevant social circumstances of which you are aware?' She replied ' Stress at work in early 2007, signed off sick and then lost job'.

    The form then asked for 3 previous addresses.


    How does that sound?
