Well this muppet managed to get her oh's dob wrong when she completed the online application and the gems at the passport office told her how to resolve the issue ....and get the new passport to us within a week.

I agree, the form is not very clear. The box is intended for a child's application where the parent (the applicant) is signing on behalf of the child or, in the case of an adult, for someone who is applying (the applicant) on behalf of somebody who cannot sign (for example for reasons of disability) explained in Note 2 - Supporting Documents.

The form could be better written to clarify this. I note some people have rung the Passport Office about this, and been told it's a 'mistake' - which goes to show that (by report) even some staff at the passport office appear to be confused by this additional box!

I write information leaflets for public use and in my experience, this is not a well designed form.
