Maybe the above is an on-line version and the OP is using a paper version?
Sort of, but I think its the other way round.
If you complete the application on line you have to print a single page yourself, sign it, and send it off with your bits and pieces.
No idea what the wording was but the form looked nothing like the section 9 pointed to in this thread.0 -
The paper version is the same as the one Powerful Rogue posted a link to, except it's brown.
The online version only presents you with the pages that you need and a declaration sheet to sign, there are no 'extra' boxes on it, and as Zedicus has said, it's nothing like the paper one.Accept your past without regret, handle your present with confidence and face your future without fear0 -

Admittedly the declaration linked to by Zedicus isn't 100% foolproof (what is!) but if I were completing it I'd only complete the two boxes on the bottom right if I were applying on "behalf" of someone else.0

The op was venting his/anger as this is the appropriate forum for that, if they had come on for advice I'm sure they would of chosen one of the others, preferably the holiday board!0

Still seems pretty straightforward to me. Even the guidance notes talk about if you are making the application on behalf of somebody else.0 -
I think its pretty obvious that you only fill those boxes in if you are applying on behalf of somebody else such as a child.
Not exactly rocket science.0 -
