What about on top of a hill? They still work there, they work *especially* well there - though there is no "backdrop"...
But at the top of a hill can 'see' multiple transmitters - I'll give you the attentuation of the skull isn't 100% - but most doesn't get through; and diffraction can happen around your head....0 -

Attenuation of your head is significant, but you have to think about which has the most water (which is the major source of the attenuation) - is it:- Your brain.
