I know you can't get a company to terminate you unless you hand in your notice.
They have had letter after letter,including my medical,I do know the underlying problem,it's money and they don't want to pay out,so ignoring is there way of avoiding,

I would say the problem is they are not understanding what it is you want and what you are asking them for.

Also can some one explain contractual notice to me,they only thing I can understand from my original contract is,
One week notice if I leave,or if dismissed up to one year for every twelve full worked,I'm currently coming up to sixteen years,0 -

So the question you want them to answer then is:
You cannot return to work due to ill health, so are they going to pay you anything? e.g holiday pay, notice etc.
