And I want to know how much the completion of this adverse reaction form is going to cost someone who has a private dentist and whether Listerine be reimbursing those costs. I have to say I think they're being outrageous.

i did this towards the end of last year.....is it a new one or just old stock?....if i remember correctly it wasnt a long running promotion and was quite difficult to find at the time
