I have written to object and sent the letter back to them in the return paid envelope they sent me with the form. There was no telephone number or email contact on the letter. Still haven't heard anything.

My dentist will not complete a long questionnaire for them without charging me, time is money, and most people do know that alcohol contains sugar - which is bad for teeth. This wasn't the only reason I gave by the way. This is personal information surely? Just out of curiosity, would you ask your dentist to complete one of these forms?

Well I have just got back from work and in the post was a letter from Johnson & Johnson. It says my refund for the Listerine is being processed and I should be receiving my cheque soon. So if you get a letter like mine, just write back to them and say you would like a refund please.

Well done! I've just bought mine, so 3 weeks to wait before I can claim. If more of us 'stand up to them'/refuse, to get their form completed, perhaps they'll back down.
