Having a door to separate it from the kitchen is one up on a flat I was asked to consider renting in Harley Street in the very early 1970s. There the bath was in the kitchen and the top covering it was designed to be part of the kitchen work surfaces.0 -

I think there used to be a building reg which meant WCs could not be directly off another room but I understand that is long gone (or I was wrong all along).
I know, in American usage, a WC is a "bathroom". However, in the house which started this conversation off, it is not a WC that is directly off the kitchen but a room with a bath.0 -
there used to be a reg that said - a washbasin and 2 doorways between a bathroom and a kitchen.. but when we joined the EU this was no longer needed (thats my understanding anyway!)0
