Tag on a dog and details to put on it?

We have just come back from pets at home where it had a notice up saying that it was law for dogs in public to have a name tag on?
So, worried, we bought one and then deliberated for ages at the machine as to what details to put on.
I said that I thought I had read that owners were advised to just put the dogs surname on and a phone number for safety (to avoid someone trying to call your dog and steal it). OH is feels the tag looks stupid with our surname on it.
Then after we got it printed with 'x family dog' and phone number underneath, we noticed a sign saying that the dog act (or whatever it is) states that you need surname, address and phone number?
We dont want to put our address on, and by then the tag had been printed.
He is chipped and we thought this was enough. Anyway, he's got his tag now.
What do others do?

Over here in N.Ireland dog licences still exist, therefore dogs have to have the corresponding tag on their collar. It works well enough, if only all owners bothered themselves to actually get a dog licence.0 -
Address wise if someone found your dog it would be much easier for them to bring it back, especially if it was just outside by your house - they wouldn't know. Legally it should be there, but it's also practical.
We have, house no. road name, town and postcode. Our name and telphone number. Not dog's name.
You can order them online quite cheaply if you want to make one with your address and keep the one without address for emergencies like slipped collar etc.0 -

I have pet name, my surname, first line of address and postcode, my phone no, vets phone no, and 'i am chipped and neutered'
I can't imagine anyone wanting to steal my pooch, but i can imagine them wanting lots of info so they can return her!lol
If you don't want to put your address or pet name, that's understandable, but I would try to include the rest of the info I have on mine eg vets no, i am chipped, neutered, and possible at least part of your postcode, in addition to family name & contact numbersI don't know half of you half as well as I should like, and I like less than half of you half as well as you deserve.

Mine have their names (they would go with anyone no matter what they called them!), house number, postcode, mobile number, and 'I am Chipped' on their tags.
And they work!
One of them has got out of the garden whilst I was at work twice - both times I got a call on my mobile within minutes.Some days you're the dog..... most days you're the tree!0 -
our dogs have my first initial and surname, door number and postcode and phone number.
they have another tag with vet details.0 -

I'm not sure where you are but the 8 areas Dog Wardens I've worked with would always call the number on the tag or ID chip.Some days you're the dog..... most days you're the tree!0 -
Why wont they release the dog until you pay the bill ?

Mine would too!
I get all my tags (pooch has got a few different styles / colours for variety!!) from eBay loads cheaper than pets at home.
